Exercise clearance for Flying Fit

This form is for a DAME or GP to confirm exercise clearance for a pilot starting a Flying Fit program. It is completed during the pilot’s appointment and takes about two minutes.

What we collect and why:

• the pilot’s name, email and AIPA member number, and

• the clinician’s name, role, AHPRA registration number, clinic details and clearance statement.

We use these only to confirm clearance and enrol the pilot in their program. We may contact the clinic to confirm the clearance.

The form holds no diagnoses, examination findings or medical history.

Details are stored securely and we share only the pilot’s name and membership number with AIPA’s membership team, to confirm the pilot’s membership. For more information, see our Privacy Policy.
